چکیده

The surgical treatment of destructive processes involving the lower lumbar spine and the sacrum often require attachment of a long hardware construct to the pelvis for improved stability. In an effort to minimize surgical morbidity and postoperative pain, and improve quicker patient mobilization, we describe a novel minimally invasive technique of percutaneously placed iliac screws. We explain the procedure in detail, while presenting the case of a patient suffering from a lytic lesion of the sacrum that was treated successfully with this method. This technique is a viable alternative to open surgery and an asset in the armamentarium of contemporary spinal surgery.

Percutaneous Iliac Screws for Minimally Invasive Spinal Deformity Surgery

Introduction. Adult spinal deformity (ASD) surgeries carry significant morbidity, and this has led many surgeons to apply minimally invasive surgery (MIS) techniques to reduce the blood loss, infections, and other peri-operative complications. A spectrum of techniques for MIS correction of ASD has thus evolved, most recently the application of percutaneous iliac screws. Methods. Over an 18 mont...

Free-hand placement of iliac screws for spinopelvic fixation based on anatomical landmarks: technical note

BACKGROUND The placement of iliac screws is a biomechanically sound method for the stabilization of long multi-segment lumbar constructs. Traditional techniques for the placement of iliac screws often involve either substantial iliac muscle dissection for visualization of screw trajectory based on bony landmarks, or alternatively the use of intra-operative imaging to visualize these landmarks a...

Miniopen Oblique Lateral L5-S1 Interbody Fusion: A Report of 2 Cases

Extreme lateral interbody fusion (XLIF) has been widely used for minimally invasive anterior lumbar interbody fusion (ALIF), but an approach to L5-S1 is difficult because of the iliac crest. In the current study, we present 2 cases using minimally invasive oblique lateral interbody fusion (OLIF) of L5-S1. The patients showed foraminal stenosis between L5 and S1 and severe low back and leg pain....

The reduction and direct repair of isthmic spondylolisthesis using the smiley face rod method in adolescent athlete: Technical note.

Presently, lumbar spondylolisthesis did not have the indication of direct repair surgery because of the difficulty to reduce the slippage. In this report, we presented a case and described a minimally invasive direct repair surgery to reduce and repair the pars interarticularis defects of lumbar spondylolisthesis. First, curettage and removal of the synovium of the pars interarticularis and dec...
